Abstract: In consideration of non-strict attribute and betweenness for drinking water source form and class attribute, nine assessment factors which have much more influence on human health were identified, for example, permanganate index, Escherichia coli, total nitrogen, total phosphorus, dissolved oxygen and so on. Through constructing the judgment matrix and consistency checking, judging the consistency of expert opinions, effective combination of qualitative and quantitative analyses, from the angle of mathematics analysis, the authors presented the numerical relationship between the comparisons of various indexes. The evaluation factor level index and fuzzy mathematics model based on AHP method combination help us to establish the city reservoir water quality fuzzy assessment model. Using water quality monitoring data from Fuxin City main water sources in 2010 the authors evaluated  the main factors affecting Fuxin City water quality, the causes of pollution, pollution levels, respectively and also conducted applicability analysis. The results showed that the main factors affecting  Fuxin reservoir water quality are the permanganate index, total phosphorus and total nitrogen in agricultural and pastoral areas. The intensive fertilizing and spraying pesticides is the main reason why reservoir's total phosphorus exceeds the standard.
